Catch chance
This page shows the chance of each ball at each rarity, worked out with the same math the game uses. It applies to a species of median difficulty, with your team at the target's level.
Below 1% the chance shows as < 1%. In that range what counts is persistence, and the exact number would not change your decision. Shiny and legendary are extremely rare with any ball.

Above Common, the Ultra Ball catches about 3 times as much as the Poké Ball. If your Poké Ball is being thrown at rare targets, reserve the Ultra for them in the ball rules of the catch panel.
What makes catching harder
The species. Every species has its own difficulty. With an Ultra Ball, on a Common target:

Rarity. The rarer, the harder (table above).
A team below the target's level. With the average of the three below 90% of the target's level, the chance drops, and fast. The full table is in Catch.
Shiny and legendary. They are extremely rare with any ball, and each shiny gives you a single throw.
Very hard species to catch
These species are Very hard to catch. This includes the starter lines, the pseudo-legendaries (like Dratini, Gible, Larvitar and Beldum) and other tough species. Legendaries are not listed: they are extremely rare. Each species' difficulty number is in Catch Rates.
